Please arrive 15 minutes early. Your first visit will be approximately 1 hour in length. We encourage you to bring your spouse or significant other to this visit. During this visit, we will provide a brief patient education overview.
The doctor will review your medical history and will perform a thorough exam. If X-rays are needed, they will be taken onsite.

We like to keep things as easy as possible for you. When you book your appointment, we will send you an email containing a link to fill your new patient intakes out online. No need for printing.
No suprise act of 2022
The No Surprise Act became effective January first 2022. It provides protection for patients from getting a “Surprise Bill” in the mail.
Our clinic will review with you a Good Faith Estimate of the charges to expect for those items or services reasonably expected to be furnished to you upon your receipt of the scheduled or requested primary item or service. If you do not understand the charges we request that you please ask. There may be additional items or services which the provider recommends as part of your course of care that you will be required to schedule separately which may not be reflected in this Good Faith Estimate. An additional estimate may be required as therapy continues. Should you require further information please request it from our staff.
